Appointments
All patients are seen by appointment. We know that when your child is sick, he or she may not be able to wait a few days, so we reserve appointment slots for sick visits every day. Well visits are more involved and need to be scheduled further in advance.
Cancellations and No-ShowsIf you are unable to make your scheduled appointment, notify us as soon as possible so that we can offer that time to another patient.
We charge a $50 fee for no-shows or last minute cancellations and reschedules. Medical RecordsImmunization records, growth charts, medication lists, labs and visit notes are available on the patient portal and can be printed by you at your convenience.
If you are unable to print from the portal, a copy of the medical record can be transferred to another practice or picked up in person from our office. You must fill out a Release of Medical Records Form, which can be found on the website under Registration / Forms. There is a $15 fee and you must allow 5 business days for processing. Records cannot be mailed to a residence. Administrative FormsWe can fill out forms needed for your child for school or daycare entry, sports participation, camp participation, etc., if they have had a well check within the last 12 months (or other time period stated on the form). There is a $15 fee for most forms. Please allow 3 days for us to process this.

Phone Calls
If you have a question during office hours, our staff will take a message to be forwarded to one of the providers. You will receive a call back with the response. Your call will be returned as quickly as possible, but is dependent on how busy the office is and how urgent the matter is.
We do not prescribe antibiotics or other medications used for acute illnesses over the phone. If your child is sick enough to require a medication to get better, he/she should be examined.
We have an after-hours triage nurse service for urgent matters, that cannot wait until the next day. There is a $20 charge per call, except for Medicaid patients. A Pediatric RN will address your concerns. If the situation is an emergency and needs immediate attention, please call 911.

Prescription Refills
Refills for medications that your child takes on a chronic basis can be refilled during an office visit or through a pharmacy request. If you have switched pharmacies, you can call us and request the prescription be sent to the new pharmacy. Please allow 3 days for us to process this.
After reviewing any refill request, we may contact you to schedule an appointment before the medication is refilled.
